Through regular contact, our case managers are involved in planning and coordination of health care services, while monitoring progress, to ensure our clients receive high quality, cost-effective care.

Case managers serve as liaisons between the individual, the health care facilities/providers, the insurance company or other pay sources and any health care resources available. This enables individuals to return to their maximum living, wellness and functional capacity and serves to benefit everyone involved.

We currently offer case management to a number of clients, among them, the brave men and women who responded to the September 11 disaster in 2001, through the World Trade Center Health Program. As an organization, we are both proud and humbled to be entrusted with this important responsibility.
